  • The Assessment of Motor and Process Skills (AMPS)

     The AMPS is an observational assessment based on MOHO that is used to measure the quality of a person's performance of domestic (instrumental) or basic (personal) activities of daily living (ADL). The quality of the person's ADL performance is assessed by rating the effort, efficiency, safety, and independence of 16 ADL motor and 20 ADL process skill items.

  • The Assessment of Occupational Functioning - Collaborative Version (AOF-CV)

     The AOF-CV is a semi-structured, self-report instrument designed to yield qualitative and quantitative information about how a person is functioning relative to key components of MOHO. It can be administered as either an interview or a written self-report. It is appropriate for use with adolescents and adults across many populations, but in particular with the elderly in in-patient settings, individuals with schizophrenia, and persons with alcohol dependence.  Therapists with thorough knowledge of the Model of Human Occupation and good interviewing skills will likely make best use of the yield for clinical reasoning.

  • The Assessment of Work Performance (AWP)

     The AWP is an observational instrument based on MOHO that is being developed in Sweden. The AWP is designed to assess the individualÂ’s working skills in three areas: motor skills, process skills, and communication and interaction skills. It determines how efficient and appropriate the client performs a work activity. The AWP can be used with clients with different kinds of work-related problems, and the assessment can be used in both real life work situations and constructed environments. The AWP is available only in Swedish at the moment.

  • Modified Interest Checklist

     The Modified Interest Checklist gathers information on a client's strength of interest and engagement in 68 activities in the past, currently, and in the future. The main focus is on leisure interests that influence activity choices. The checklist can be used by adolescents or adults.

  • UK Modified Interest Checklist

     The UK Modified Interest Checklist gathers information on a client's strength of interest and engagement in 74 activities in the past, present and future. Interests are listed in nine categories that focus on different types of activity choices. The checklist can be used by adolescents or adults.

  • NIH Activity Record (ACTRE)

     The NIH Activity Record documents an individual's participation in occupations by half- hour intervals throughout the day. Users categorize the activity and record pain, fatigue, level of difficulty, perception of competence, value, and enjoyment for each activity. The questionnaire can be used by adolescents or adults.

  • Occupational Questionnaire

     The Occupational Questionnaire documents an individual's participation in occupations by half-hour intervals throughout the day. This is a shortened version of the NIH Activity record. Users classify the activity as work, play, or leisure, and record their perception of competence, value, and enjoyment for each activity. The questionnaire can be used by adolescents or adults.

  • Occupational Therapy Priority Checklist
  •      The OT Priority Checklist has been developed by clinicians in the UK and is designed to be completed in discussion with the wider multi disciplinary team at the point of referral. It has proved to be a useful a starting point for screening referrals to occupational therapy and introducing the framework of the Model of Human Occupation, paving the way for more in-depth assessment of occupational function.

    • Pediatric Interest Profiles

         Clients answer questions about their interest and participation in a variety of play and leisure activities. Three activity scales, the Kid Play Profile, The Preteen Play Profile, and the Adolescent Leisure Interest Profile, make this self-report/ interview profile appropriate for clients ages 6-21.

    • Residential Environment Impact Scale

          The Residential Environment Impact Survey (REIS) is a non-standardized, semi-structured assessment and consulting instrument designed to examine the environmental impact of community residential facilities on the residents. Using four different methods of data collection, the REIS assesses how well the home is meeting the needs of the residents as a whole. Findings from the REIS can be used to guide intervention aimed at modifying the residential environment. While designed with adults with mild to moderate intellectual disabilities in mind, the REIS can be adapted to other residential settings.

    • Role Checklist

         The Role Checklist was designed to obtain information on client's perceptions of their participation in 10 occupational roles throughout their life. It also assesses the value they place on those occupational roles. The checklist can be used by adolescents or adults.

        The Model of Human Occupation (MOHO) is the most frequently used occupation-based model in the field of occupational therapy.     • Conceptual Foundations of Occupational Therapy (3rd edition)

         Conceptual Foundations of Occupational Therapy (3rd edition) provides readers with a method of evaluating models for practice. The text includes an extensive history of the field of occupational therapy, a discussion and comparison of eight well-known models of occupational therapy practice, including MOHO, and an introduction to interdisciplinary bodies of knowledge contributing to the profession.

    • MOHO Center in Taiwan
      The MOHO center in Taiwan was instituted in 2006 with the support by Dr. Gary Kielhofner at the University of Illinois at Chicago.
